Liability Waiver

By accepting this agreement, I attest that the participant(s) listed are in good physical condition and able to fully participate in activities at Core Taekwondo.

I understand that the school, its owners, operators, instructors, staff, volunteers, and persons in charge shall not be held liable for any injuries, illness, loss, theft, or damages that may occur while participating in activities or while on the premises. Participation in all activities is voluntary and at the participant’s own risk.

I further authorize the school and its agents, successors, and assigns to photograph, video record, or otherwise capture images and/or audio of myself and/or my child(ren) without restriction and to use such materials for promotional, advertising, marketing, social media, or other commercial purposes related to the school. I understand and agree that no compensation of any kind shall be provided for such use.

Health & Safety

I understand that contagious illnesses, including COVID-19 and other communicable diseases, may be spread through person-to-person contact.

I agree that all members of my family will follow applicable health and safety guidelines while participating in activities at Core Taekwondo.

I understand that if any member of my family is experiencing symptoms of illness, including fever, cough, vomiting, or other contagious symptoms, they should not attend in-person activities until they are no longer contagious.

I acknowledge that while reasonable cleaning and safety measures may be taken, Core Taekwondo cannot guarantee that participants will not be exposed to illness while attending activities.
